BETWEEN: Care For Kidz Nanny Agency Pty Ltd ACN 092 342 464 (“Care For Kidz”) AND The undersigned (“you”)

1. Care for Kidz provides parents/guardians with the name and contact details of a another parents/guardian with the purpose of
sharing child care costs.

2. you the undersigned acknowledge that Care for Kidz is acting only as a introductory agent only and the final decision is that
of you.

3. you acknowledge that it is you who is obligated to notify the other parent of any disabilities, special needs illnesses current or
future that may affect the parents/or other children at all times.

4. You will pay Care for Kidz the registration fee specified in Item 1 or this Schedule.

5. You acknowledge that this registration fee is non-refundable (except under clause 6) The registration is valid 12 weeks from
the date of formation of this agreement.

6. Care for Kidz will only give a full refund if Care for Kidz can not introduce you to another parent/guardian to share child care
costs within the 12 weeks from date of receipt of this agreement.

7. You hereby release to the full extent of the permitted law, Care for Kidz and/ or their servants/agents, trading as Care for
Kidz from all claims and demands with respect to any accident, damage, loss to person and property, pain and suffering
however caused.

8. You wholly indemnify Care for Kidz from and against any actions, suits, demands, claims, cost, damages, and expenses to
which Care for Kidz are, or may be liable.

9. You hereby elected to participated in sharing child care costs with a another parent/guardian organised through Care for Kidz
and do not hold Care for Kidz or the people involved in Care for Kidz responsible for and hereby indemnify Care for Kidz
from any personal injury, loss or damage which may result from you participation.
10. Care for Kidz does not accept any liability for any loss or injury suffered or incurred by you or any of your children or
damage to you, any of your children or your property directly or indirectly under this agreement and you agree to indemnify
and keep indemnified Care For Kidz with respect to any such loss or injury or damage whether or not caused directly or
indirectly by any act, omission or negligence of Care For Kidz of any of its servants or agents. This release and indemnity
continues forever.

11. Either party may terminate this agreement by notice in writing to the other.

12. All notices and other communication required to be given under this agreement must be in writing and must be addressed to
the principal place of business of the applicable party or such other address as that party may have notified the other party in
writing, and shall be either be delivered personally or by pre paid post.
